I currently collect and process the following information:

vi. Current mental health assessment by questionnaire and discussion.

xi. Relationship(s) and family details.
How the personal information is obtained and why it is necessary
1. Information is obtained by questionnaires, interview and discussion.
2. The information is used to:
i. Understand client (or clients for couple therapy) requirements.
ii. To assess current mental health.
iii. To assess and manage risk.
iv. To inform the direction of psychological therapy.
3. This information may be shared in whole or part:
i. For the provision of reports required by medical insurers.
ii. For the provision of reports required for legal reasons.
iii. To manage risk of harm to self or others.
Under the General Data Protection Regulation (GDPR), the lawful bases for processing this information are:
1. Your consent. You are able to remove your consent at any time. You can do this by contacting Peter Gregory as above.
2. To enable the provision of psychotherapy.
3. I have a legitimate interest (to provide the service of psychotherapy).
4. Reasons personal information may be passed to a third party:
i. If there is a risk of harm to self or others. I am legally bound to take appropriate action. Depending on circumstances, I may not advise you I have taken this action.
ii. If instructed to do so by a court of law.
iii. If required by a medical insurer or medical professional. In these circumstances, your written permission will be required before any information is disclosed.
iv. Client related reasons; in these circumstances, your written permission will be required before any information is disclosed.
How your information is stored:
1. Electronically and paper records.
2. To meet the requirements of my professional body, records are kept for 7 years.
3. Paper records are destroyed using a GDPR compliant shredder.
4. Electronic records are destroyed by means of an eraser program.
Your data protection rights
1. Your right of access: You have the right to ask for copies of your personal information.
2. Your right to rectification: You have the right to ask me to rectify personal information you think is inaccurate. You also have the right to ask me to complete information you think is incomplete.
3. Your right to erasure: You have the right to ask me to erase your personal information in certain circumstances. Please note this may affect your rights in respect to complaints.
4. Your right to restriction of processing; You have the right to ask me to restrict the processing of your personal information in certain circumstances.
5. Your right to object to processing: you have the right to object to the processing of your personal information in certain circumstances.
6. Your right to data portability: You have the right to ask that I transfer the personal information you gave me to another organisation, or to you, in certain circumstances.
You are not required to pay any charge for exercising your rights. If you make a request, I have one month to respond to you.
